ROLE SUMMARY

We are seeking a highly skilled and dedicated Pharmacist to join our Company. The role will be responsible for managing our Hospital Pharmacy that serves over 5,500 employees and their dependants, including the relevant surrounding communities. As a Pharmacist, you will be responsible for preparing and administering appropriate pharmaceuticals to patients. You will achieve this either by executing a physician’s order or by addressing the patient’s problems and needs. The goal is to ensure effective medication use and become the people’s trusted ally against health issues.

MAIN ACCOUNTABILITIES

Medication Dispensing:

 Dispense prescription medications accurately, ensuring proper dosage, labeling, and instructions.
 Verify and clarify medication orders with prescribers when necessary. 
 Review and execute physician’s prescriptions checking their appropriateness and legality.
 Provide counselling to patients on the proper use of prescribed medications, potential side effects, and interactions.

Medication Management:

 Collaborate with healthcare providers to review and optimize medication therapy.
 Monitor patient profiles to prevent adverse drug reactions and ensure therapeutic effectiveness.
 Adjust medication regimens as needed based on patient response and laboratory results.
 Keep records of patient history and of all activities regarding heavy medication

Patient Education:

 Educate patients on the use of over-the-counter medications and supplements.
 Provide information on lifestyle modifications and adherence to medication regimens.

Compounding:

 Prepare and compound specialized medications as required, adhering to safety and quality standards.
 Ensure proper documentation and labeling of compounded medications.

Medication Safety:

 Participate in medication safety initiatives and error prevention strategies.
 Report and investigate medication errors, adverse drug reactions, and near misses.
 Listen carefully to customers to interpret their needs and issues and offer information and advice

Inventory Management:

 Manage and maintain an organized and efficient pharmacy inventory.
 Monitor medication expiration dates and coordinate returns and disposal as necessary.

Process Controls & Quality improvement: 

 Define Processes and Identify Key Performance Indicators (KPIs) at the Pharmacy 
 Implement Quality Management Systems (QMS)
 Establish Continuous Quality Improvement (CQI) Teams
 Conduct Regular Audits and Assessments

Regulatory Compliance:

 Ensure compliance with local pharmacy laws and regulations.
 Stay updated on changes in pharmacy practice and regulations.

Collaboration with Healthcare Team:

 Work collaboratively with physicians, nurses, and other healthcare professionals to optimize patient care.
 Participate in interdisciplinary rounds and meetings as required.
 Keep abreast of advancements in medicine by attending conferences and seminars and collaborating with other healthcare professionals

QUALIFICATIONS, KNOWLEDGE, SKILLS & EXPERIENCE REQUIRED

 Bachelor of Pharmacy (B.Pharm) degree from an accredited school of pharmacy with a curriculum acceptable to the Pharmacy and Poisons Board .
 Must be registered or registrable to the Pharmacy Regulatory Authority. 
 Previous experience in a Hospital set up is highly preferred
 Proven experience as a Pharmacist. Meaningful experience post Pharmacy internship is preferred
 Strong knowledge of pharmaceuticals, drug interactions, and pharmaceutical care principles.
 Excellent communication and interpersonal skills.
 Attention to detail and accuracy in medication dispensing and record-keeping.
 Residency or additional certifications in a specialized area of pharmacy practice.
 Familiarity with pharmacy software systems.
 Ability to work in a fast-paced environment and handle multiple priorities.
